Poe's narrator goes out of his way to let us know he's not insane. He seems to be possessed by guilt and is fixated on the old man's eye.

He seemed to be waiting to kill the old man, but we're never really sure why. The old man's eye seems to represent his guilt and the beating of the old man's heart as the killer's anxiety and madness. He believes he needs to take the old man's life and believes it is the only thing that will make the paranoia stop. He kills the old man and burries him under the floor boards.Burying his guilt once and for all.
When the police show up, he believes he can be cool. He's over-confident and invites them in, almost challenging fate. But then the heart starts beating again. His subconscious is working him into a frenzy. It's so strong he believes the police officers can even hear it and reveals himself as the killer.
An insane man is sane in his own mind.
